Section Q addresses discharge planning and the resident’s desire to return to the community. The Office of the State Long-Term Care Ombudsman has created a fact sheet for long-term care facilities that address the roles of each participating entity when making a Section Q referral; determining whether a Section Q referral is appropriate; the steps to take when making a Section Q referral; and follow-up requirements.
